benefits and get you scheduled.  We are here to serve YOU, start to finish.


*Please note we do not accept ANY type of Medicare or Medical Card/Public Aid. 


**If you carry Blue Cross Community Health Plans, this is a form of Medicaid and we cannot bill them. Please refer to your Blue Cross member ID; if it begins with the prefix XOG or XOJ, this is a Medicaid plan.


**We CANNOT accept Health Partners at this time, even though they are a subsection within Unity Point. We apologize for any inconvenience.

EAP

An employee assistance program (EAP) is an employee benefit program offered by many employers. EAPs are intended to help employees deal with personal problems that might adversely impact their job performance, health, and well-being. EAPs are designed as short term counseling options and are often free for employees and their family members.


We are only accepting District 150 Peoria Public Schools at this time. Even if you have an EAP benefit, we can initiate counseling through your insurance plan.

Self-pay/Uninsured Clients

“NO SURPRISES ACT” GOOD FAITH ESTIMATE NOTICE

You have the right to receive a “Good Faith Estimate” explaining how much your care will cost. Under the law, licensed mental health professionals need to give clients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.


  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. 


• Your provider must give you a Good Faith Estimate in writing for scheduled services within designated timeframes. You can also ask your provider for a ‘Good Faith Estimate’ before you schedule an item or service. 


• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 


• Make sure to save a copy or picture of your Good Faith Estimate.

EAP Peoria Public School District 150

We are both proud and honored to announce that we are now an approved EAP provider for Peoria Public School District 150 employees and their immediate family. This allows employees and their family CONFIDENTIAL access to FREE and UNLIMITED counseling with us. Please call us at (309) 966-0068, and ask us how we can help set this up for you. Or, you can contact Human Resources at PPS District #150 and they can lead you in the right direction to get help with us.
