**If you carry Blue Cross Community Health Plans, this is a form of Medicaid, and we cannot bill them. Please refer to your Blue Cross member ID. If it begins with the prefix XOG or XOJ, this is a Medicaid plan.

An employee assistance program (EAP) is an employee benefit program offered by many employers. EAPs are intended to help employees deal with personal problems that might adversely impact their job performance, health, and well-being. EAPs are designed as short term counseling options and are often free for employees and their family members.
We are ONLY accepting District 150 Peoria Public Schools EAP at this time. Even if you have an EAP benefit, we can initiate counseling through your insurance plan and skip you directly to using your insurance benefits.
You have the right to receive a “Good Faith Estimate” (GFE) explaining how much your care will cost. Under the law, licensed mental health professionals need to give clients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
• You have the right to receive a GFE for the total expected cost of any non-emergency items or services.
• Your provider must give you a GFE in writing for scheduled services within designated timeframes. You can also ask your provider for a GFE before you schedule an item or service. We provide a GFE upon scheduling your first visit which can be viewed within your patient portal or sent ahead of time upon request.
• If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
• Make sure to save a copy or picture of your GFE.
